Medical Records
Notice to Patients Requesting Copies of Their Medical Records:
- The Health Information Services Department is open 8 am to 3:30 pm, Monday through Friday. You can reach us by calling 503-261-6654.
- Fill out an Authorization to Release Medical Information Form. Download and print the Authorization to Release Medical Information form (PDF) or you can pick it up at our Health Information Services Department in the medical center.
- Mail your request to: 10123 SE Market Street, Portland, OR 97216, ATTN: Health Information Services or Fax to: 503-251-6259.
- Personal record copies are subject to a processing fee. Upon request, we will call you to discuss the copy fee once we have your records so you are aware of the total cost.
- We can mail your records or you can pick them up if you prefer. You will not be charged for postage.
- Please allow 3-5 working days to process your request.
NOTE: Records are sent to your doctors at no charge.
