Notice to Patients Requesting Copies of Their Medical Records:
- The Health Information Services Department is open 8 am to 3:30 pm, Monday
through Friday. You can reach us by calling 503-251-6266 ext. 7284.
- Fill out an Authorization to Release Medical Information Form. Print the
form here
or you can pick it up at our Health Information Services Department in the
medical center.
- Mail your request to: 10123 SE Market Street, Portland, OR 97216, ATTN:
Health Information Services or Fax to: 503-251-6259.
- Personal record copies are subject to a processing fee. Upon request, we
will call you to discuss the copy fee once we have your records so you are
aware of the total cost.
- We can mail your records or you can pick them up if you prefer. You will
not be charged for postage.
- Please allow 3-5 working days to process your request.
NOTE: Records are sent to your doctors at no charge.
