The multi-disciplinary Adventist Health/Hospice team, with over 315 years of
collective hospice experience, can assist and support your loved one and family
24 hours per day 7 days a week as they cope with the demands and challenges of
care giving. Each team member plays an important role in addressing the
physical, emotional, mental, and spiritual needs of patients, their families and
caregivers.
All hospice staff work closely with the patient’s physician to make sure that
medication, therapies, and procedures are designed to achieve the goals outlined
in the patient’s care plan which is regularly reviewed and updated.
Adventist Health/Hospice staff is also available to provide “information only visits”
to anyone who may seek further information before making a decision to elect
Hospice care.
The hospice team consists of the following:
Medical Director:
Dr. Tanya Stewart
Dr. Stewart was born and raised in the suburbs of Los Angeles, California. She
completed her undergraduate work at the University of San Diego, double majoring
in Biology and Religious Studies. Immediately after graduation, she headed to
Tufts University, School of Medicine in Boston Massachusetts where she received
her medical degree. From Boston she found her way to the Pacific Northwest
completing her internship and residency at Legacy Good Samaritan and Emmanuel
hospital in Portland, Oregon. She realized her passion for end of life care
during her residency and in 2004, she accepted a position as medical director
for Adventist Health Hospice, becoming board certified in palliative medicine in
2006.
She has a passion for teaching and has lectured at national and statewide
conferences on a variety of palliative care topics. Dr. Stewart is a Clinical
Assistant Professor at OHSU, where she mentors medical students and resident
physicians through didactic sessions and patient home visits. She is also an
instructor in Oregon Health and Science University’s 1st year medical student
course on patient’s experiencing a life limiting illness.

Clinical Medical Director:
Dr. Scott Smith
Dr. Scott Smith is a Family Physician with 20 years of experience in primary
care medicine prior to joining the Adventist Hospice team. As the Clinical
Medical Director, his emphasis is the medical management of end-of-life care.
Dr. Smith visits patients in their homes and other residential settings in order
to optimize care in collaboration with referring and primary care physicians.

Barbara L. Crane, RN
Barbara heads our team of dedicated professionals, providing them with excellent
leadership and ongoing training to ensure our standard of excellence in care.

Hospice Patient Care Coordinator:
Stephanie Templeton, RN, CHPN
The patient care coordinator is responsible for support and the direction of the
clinical staff to ensure that they are orientated, educated and providing the
highest care possible.

Hospice Intake and
Triage Nurse
The Hospice intake and triage nurse is responsible for managing patient care
services and clinical staff activities. This busy position coordinates
referrals; receives triages & manages all incoming calls, oversees patient care
assignments and staff scheduling.

Nurse
The Hospice Nurse visits the patient’s home regularly and serves as the eyes and
ears of the physician. He/she performs and/or instructs the patient and
caregivers on all aspects of care-giving, symptom control, comfort measures,
safety concerns, and administration of medications. The nurse is accessible 24
hours a day for patients and families and to communicates with the physician as
necessary.

Social Worker
The Hospice Social Worker assesses and addresses psycho-social issues, offers
emotional support, counseling, assistance with community support services, and
arranging end-of-life plans.

Home Health Aide
The Home Health Aide assists the patient with bathing, personal care and light
housekeeping service, checking vital signs and reporting any significant changes
in the patient's condition.

Music Therapies
The Music Therapist is trained to provide comfort through music and distract
patients from physical pain and provide emotional comfort and reduction of
anxiety, agitation, depression, insomnia, and other symptoms. Music therapy is
the use of musical or rhythmic interventions specifically selected by a music
therapist to effect positive changes in the psychological, physical, cognitive
or social functioning of individuals. The goal of Music Therapy is to utilize
the elements of music as a non-invasive intervention to address physical,
psychological, cognitive and/or social functions for the hospice patients.

Music Thanatologolist
Music Thanatology is defined as a palliative medical/spiritual modality
employing prescriptive music to tend to the complex physical and spiritual needs
of the dying. The prescriptive application of music by a trained music
Thanatologist involves tailoring musical elements to the patient’s presenting
physiology, thereby providing pain relief, symptom management and physical,
emotional and spiritual support at the end of life. The goal of a music
Thanatology harp vigil is to utilize music as a non-invasive intervention to
address physical, emotional and spiritual needs of the hospice patient.

Pet Therapy
Adventist Health Hospice collaborates with Oregon Humane Society for pet therapy

Physical Therapist
The Physical Therapist helps the patient to adapt to changing levels in strength
and endurance to maintain mobility and independence as long as possible. They
can also assist with pain management using ultrasound, massage, exercise, and
relaxation techniques.

The Speech Therapist assists the patient in identifying and
addressing speech disorders and helps with the enhancement of communication and
addresses swallowing or other upper aero digestive disorders.

Chaplain
The Hospice Chaplain assists the patient in identifying and addressing
spiritual concerns and is available to support both patient and family. The
Hospice Chaplain supports the spiritual needs of patients by assisting the
patient and their family deal with changes and challenges and help cope with
process of dying. The chaplain is there to listen and offer spiritual support no
matter the particular religious beliefs, and does not proselytize or try to sway
the patient or family to any particular faith.

Volunteer Coordinator
The Volunteer Coordinator recruits and trains our volunteers. She/he supports staff and patients matching skills and talents to appropriate tasks. Volunteers* are cherished members of the Hospice team, working together to provide varied services to our patients and clients. There is always room for another caring person to help with our team! *Screened and trained Hospice Volunteers provide a variety of services including companionship and respite care up to four hours per week. These services can include respite, shopping, friendly visits, light house-keeping, running errands, reading to the patient, playing games, mailing letters, assisting with personal grooming, and recording the patient’s life-stories.

Bereavement Coordinator
Bereavement coordinator provides up to 13 months of support to the families of our patients through home visits, grief classes, bereavement support groups, letters, literature and phone calls following the death of their loved one. Our bereavement services are open to all community members dealing with loss, free of charge.
